We have Charm PHR as our go-to place for your Patient Health Record (PHR). Charm PHR is HIPAA-compliant and upholds rigorous privacy and security measures to keep your health information safe and sound. This user-friendly platform provides a simple interface for you to manage all health information online. You can securely share lab records, complete online forms, or schedule appointments with Dr. Hsieh via your CharmHealth EHR accounts. After your appointment, Dr. Hsieh can share visit summaries and handouts with you through the Portal for quick access.

With Charm PHR, you can:

• Add and manage health records like allergies, clinical problems, and other medical histories.

• Share your health records with Dr. Hsieh through the mail.

Click here to access your patient protal. New patients will receive a welcome email with an invitation to sign in to your account. You can log in using the email address or mobile number you provided. For added security, you will be asked to enter your password and verify your identity.
